Stelarc, Third Hand, Completed in 1980, used from 1980-1998, Aluminium, stainless steel, acrylic, latex electronics, electrodes, cables and battery pack.  Stelarc believes that the human body is destined to become obsolete, and our salvation as a species is dependent upon augmenting our existing selves using technology.  Third Hand was built to the scale of his existing arm, and is controlled using electrodes attached to his back muscles.  As you can see, he even taught himself to write with it, proving its usefulness as an appendage.
